I authorise the Centre Director to be in charge of consent, where it is impractical to communicate with me, for the child to receive medical or surgical treatment as may be deemed necessary. “I grant permission for my child/children to attend excursions to The Museum of Tropical Queensland and Billabong Sanctuary. *As per Sun Bus standards & regulations, not all buses have fixed seatbelts. For the buses that have seatbelts - ABV staff will ensure that children are seated with a seatbelt secured. For the buses that do not have fixed seatbelts, ABV Staff & the Sun bus Driver will ensure that the appropriate number of children will be assigned to each seat they will be reminded of the safety requirements.

LOCATION: ABV Care Centre, 569 Bayswater Rd, Mount Louisa, 4814 PURPOSE: For children to be encouraged to get creative with lots of fun and inclusive activities. ACTIVITY: Garden building, Mosaic art, flower pot paiting, Outdoor music wall. Wednesday 20th September - Muesum Excursion
LOCATION: Museum of Tropical Queensland, 70-102 Flinders St, Townsville City QLD 4810 PURPOSE: For children to have involvment of different interests based across history, and science. TRANSPORTATION: Bus COST: $25.00 RATIO: 1:5 approx, 15 leaders to 75 children PERSON IN CHARGE: Melissa White and ABV Care Staff RISK ASSESSMENT: Available upon request
Thursday 21st September - Wet 'n' Wild
LOCATION: ABV Care Centre, 569 Bayswater Rd, Mount Louisa, 4814 PURPOSE: To encourage exploration and learning through water play activities. ACTIVITY: Water gun, water balloon fight, Water slide, sensory tubs (foam, glitter etc) COST: NIL RATIO: 1:15 approx, 5 leaders to 75 children PERSON IN CHARGE: Melissa White and ABV Care Staff RISK ASSESSMENT: Available upon request

LOCATION: ABV Care Centre, 569 Bayswater Rd, Mount Louisa, 4814 PURPOSE: For children to be involved in and enjoy experiencing the culture of our Aboriginal and Torres Strait Islander people through dance, art and music. ACTIVITY: Bin ball, Golden child, Capture the flag, Soccer, Head, shoulders, knees and toes, Musical chairs COST: $5 FOOD: Sasuage sizzle provided RATIO: 1:15 approx, 5 leaders to 75 children PERSON IN CHARGE: Melissa White and ABV Care Staff RISK ASSESSMENT: Available upon request
Wednesday 27th September - Billabong Sanctuary
LOCATION: 2 Muntalunga Dr, Nome QLD 4816 PURPOSE: For children to be able to learn and experiance through hands on wildlife. ACTIVITY: Tours, animal information presentations, feeding the native wildlife. Friday 29th September - Disco Party
LOCATION: ABV Care Centre, 569 Bayswater Rd, Mount Louisa, 4814 PURPOSE: To encourage exploration and learning through various fun, musical experiences. ACTIVITY: Silent Disco, Party games, party food for morning tea FOOD: Various party foods. COST: $5 RATIO: 1:15 approx, 5 leaders to 75 children PERSON IN CHARGE: Melissa White and ABV Care Staff RISK ASSESSMENT: Available upon request
