I grant permission for my child/ren to participate in the below incursion/excursions. I authorise the Director/Centre Manager in charge consent, where it is impractical to communicate with me, for the child to receive medical or surgical treatment as may be deemed necessary. “I grant permission for my child/ren to attend the excursions and to travel on the bus when needed. I give permission for my child to be given snacks and meals when provided and allergy options will be given.

*As per Campbells Coaches standards & regulations, not all buses have fixed seatbelts. For the buses that have seatbelts - ABV staff will ensure that children are seated with a seatbelt secured. For the buses that do not have fixed seatbelts, ABV Staff & Campbells Coaches Driver will ensure that the appropriate number of children will be assigned to each seat they will be reminded of the safety requirements.

THURSDAY 22 September 2022 $10 Basketball Boot Camp
LOCATION: ABV Care Centre & Calvary Sports Centre 569 Bayswater Rd, Mount Louisa QLD 4814 PURPOSE: For children to be team players and develop gross motor skills. ACTIVITY: Basketball Clinic with TSV heat Players. COST: $10 RATIO: 1:15 approx, 15 leaders to 75 children PERSON IN CHARGE: Allyson Cotter and ABV Care Staff, run by TSV Heat Players. FRIDAY 23 September 2022 $5 Wet and Wild Day
LOCATION:ABV Care Calvary Bayswater Road Mt Louisa 4814 PURPOSE: for children to participate in active activities and enjoy new experiences ACTIVITY: Water slides, Slip and slides, water blasters, Water bombs game, water dunking.
