To secure your child’s place in the Easter 2021 Vacation Care Program. Please complete and sign the vacation care booking form. ALL vacation care bookings and incursion/excursion forms must be completed using the online Vacation Care Forms. Once your child/ren has been booked in and you have completed your incursion/excursion form, you will receive an email confirming your child’s booking. If you do not receive a confirmation email please contact ABV Care management, as your booking will not have been received. Bookings for the Easter 2021 Vacation Care will close Wednesday 17 March 2021 at 5pm. All bookings made after this time will incur a $10.00 late booking fee per child per week for each booking made. Please refer to the booking & cancellation information sheet for all other charges. Please note: 1. Your child/ren may be photographed whilst attending ABV Care, please ensure all permissions on your Family Enrolment Form are up to date. 2. All Incursion & Excursion forms must be completed & returned prior to your child attending the incursion/excursion. All incursion & excursion costs are added to your statement. 3. No food or products containing Nuts/Tree Nuts are to be brought into the ABV Care centre. 4. Children will arrive by 9am each morning (unless otherwise notified) & be collected by 6pm each afternoon. 5. By signing this booking form, you agree to all the fees and charges outlined in the Bookings & Cancellations information sheet. 6.You are required to go to your Mygov account and confirm your enrollment to receive CCS payments if your child/ren have not attended care in our Centre for 10 weeks. I hereby agree with the above booking information and requirements. I request my child/ren be booked in for the Easter 2021 Vacation Care Program.
